Indications/Uses
Risperdal Schizophrenia including 1st episode psychoses, acute schizophrenic exacerbations, chronic schizophrenia & other psychotic conditions in which +ve &/or -ve symptoms are prominent. Alleviates affective symptoms associated w/ schizophrenia. Maintenance of clinical improvement during continuation therapy in patients who have shown an initial treatment response. Moderate to severe manic episodes associated w/ bipolar disorders. 1-mg tab & oral soln: Short-term symptomatic treatment (up to 6 wk) of persistent aggression in conduct disorder in adolescents & childn ≥5 yr w/ subaverage intellectual functioning or mental retardation diagnosed according to DSM-IV criteria. Moderate to severe manic episodes associated w/ bipolar disorders. Risperdal Consta Schizophrenia & schizoaffective disorders. Monotherapy or adjunctive therapy to lithium or valproate for maintenance treatment of bipolar I disorder.
Dosage/Direction for Use
Risperdal tab/oral soln Schizophrenia Adult Initially 2 mg daily, may be increased to 4 mg on 2nd day. Maintenance: 4-6 mg daily. Max: 16 mg daily. Elderly ≥65 yr Initially 0.5 mg bd, may be adjusted w/ 0.5 mg bd increments to 1-2 mg bd. Manic episodes in bipolar disorder Adult Initially 2 mg daily, may be increased to 6 mg on not >1 mg dosage increments daily. May be administered in flexible doses over range: 1-6 mg daily to optimize each patient's level of efficacy & tolerability. Conduct disorder Childn & adolescent 5-18 yr, ≥50 kg Initially 0.5 mg once daily, may be adjusted by 0.5-mg increments once daily not more frequently than every other day. Dose range: 0.5-1.5 mg once daily, <50 kg Initially 0.25 mg once daily, may be adjusted by 0.25-mg increments once daily not more frequently than every other day. Dose range: 0.25-0.75 mg once daily. Not recommended in childn <5 yr. Risperdal Consta powd for inj Schizophrenia & schizoaffective disorders Adult 25 mg IM every 2 wk. Some patients may benefit from higher doses of 37.5 mg or 50 mg. Max: 50 mg every 2 wk. Upward dose adjustments should not be made more frequently than every 4 wk. Elderly 25 mg IM every 2 wk. Bipolar disorders Adult 25 mg IM every 2 wk, some patients may benefit from high dose of 37.5 or 50 mg.
Administration
May be taken with or without food.
Special Precautions
For elderly patients treated w/ Risperdal Consta, follow recommended dosage. Oral Risperdal (or another antipsychotic medication) should be given w/ 1st inj of Risperdal Consta & should be continued for 3 wk to ensure that adequate therapeutic plasma conc are maintained. Discontinue use if tardive dyskinesia/extrapyramidal symptoms or NMS occur. Known CV or cerebrovascular disease; clinically significant low WBC or drug-induced leukopenia/neutropenia; risk factors for VTE; Parkinson's disease & dementia w/ Lewy bodies; hyperglycemia & DM; wt gain; history of cardiac arrhythmias, congenital long QT syndrome; priapism; seizures or conditions lowering seizure threshold. Intraoperative floppy iris syndrome. Antiemetic effect. Conditions contributing to core body temp elevation or dehydration. Concomitant use w/ furosemide; psychostimulants eg, methylphenidate; drugs prolonging QT interval. May affect ability to drive & use machines. Renal or hepatic impairment. Pregnancy. Not to be used during lactation. Not recommended in childn <18 yr w/ schizophrenia; childn <5 yr w/ conduct disorder. Elderly w/ dementia.
Adverse Reactions
Risperdal Insomnia, anxiety; parkinsonism, akathisia, somnolence, headache, sedation, dizziness; UTI; vomiting; fatigue. Risperdal Consta Wt gain, headache, parkinsonism, dizziness, inner restlessness, depression, extrapyramidal symptoms.
Drug Interactions
Antagonized effect of l-dopa & other dopamine agonists. Emergence of extrapyramidal symptoms w/ psychostimulants eg, methylphenidate. Hypotension w/ antihypertensives. Increased plasma conc w/ strong CYP2D6, CYP3A4 &/or P-gp inhibitors eg, itraconazole, ketoconazole, phenothiazines, β-blockers, Ca-channel blockers eg, verapamil, fluoxetine, paroxetine, TCAs, sertraline. Decreased plasma conc w/ strong CYP3A4 &/or P-gp inducers eg, rifampicin, carbamazepine. Potentially raised conc w/ ritonavir & ritonavir-boosted PIs. Increased mortality in elderly w/ furosemide. Increased bioavailability by H2-receptor antagonists eg, cimetidine, ranitidine. Centrally-acting drugs, alcohol; drugs prolonging QT interval.
